How to connect a Facebook Ad form to OutPerform?

Add a Facebook Form

1. Login to OutPerform account and go to Settings page

2. Click on Facebook Forms from the left panel
   

   

3. Add Form
   

   – Click on Add Form button and type in the form name
   – Click on active/inactive label to activate or deactivate a form
   – Click on pencil icon to edit the form
   – Click on bin icon to remove the form
   
   

4. Edit Form
   

   – Select the Facebook page from the drop down
   – Select the Facebook form from the drop down
   – Select the deal source for the form
   – Select the team the lead will be routed/distributed
