How to re-connect the Facebook Form

1. Go to Settings > Facebook Form.
2. Click on the pencil button to edit the form.
3. If you notice the FB Page and FB Form are blank, it means the form is disconnected.

4. Select the FB Page from the dropdown and select the FB Form from the dropdown.

To check which FB Forms you want to connect, you can check the Form ID.
